I think I need Physical Therapy. Can I just call for an appointment?
In most cases, we will only treat patients who are referred by a doctor. If you have not yet seen a doctor, but feel you could benefit from Physical Therapy, call or visit your doctor and ask for a prescription for Physical Therapy (as well as an insurance referral if your insurance requires one). We will work closely with your doctor to provide you with a comprehensive treatment plan.

How do I make an appointment?
Call the clinic most convenient to your location (work, home, etc.). The clinic receptionist will schedule your initial appointment within 24-48 hours. Same day appointments are often available. If you need to cancel and reschedule an appointment for any reason, please provide us with 24 hours notice. Also, please remember to bring your insurance information, prescription from the doctor, and insurance referral (if applicable) to your initial visit.

What can I expect at my first appointment?
For your first visit, please bring your insurance card, prescription and referral (if your insurance requires one) from your doctor, and wear clothes you can comfortably move around in. Your physical therapist will listen and ask questions regarding the nature, history, etc. of your pain. The therapist will then perform a physical evaluation to further determine the source of your pain. A specialized treatment plan will be determined based on the results of this exam, and initiated at this visit. The first visit usually lasts approximately 1 hour. Please arrive 10-15 minutes early to fill out paperwork.

Do you accept my insurance?
We participate with most insurance carriers including: Aetna, APA Partners Health, Blue Shield of NENY, CDPHP, Empire BC/BS, Blue Choice, Healthnet, Empire Plan/United Healthcare, GHI/GHI HMO, Medicare, MVP, No Fault, and Wellcare, Worker’s Compensation. We will bill your insurance company for services rendered. If your insurance plan requires a co-pay, you will be responsible for that co-pay at each visit. If you do not have insurance, we offer treatments at a private pay rate. If you have trouble meeting the financial requirements, payment plans are available. We will work with all patients to help minimize their costs.

What is the difference between a ‘PRESCRIPTION’ and a ‘REFERRAL’?
A prescription is a written intent for you to receive Physical Therapy given to you by your doctor. The prescription must include your name, diagnosis, doctor’s signature, and date. Many doctors will also write the number of visits they would like you to receive (frequency). You must bring your prescription to your initial visit. An Insurance Referral is a form that is required by some insurances. This is also given to you by your doctor’s office. If your insurance requires you to have a referral, you may bring the referral to your initial visit, or have your doctor’s office fax it to us.

We have hours available Monday - Friday, 7.30am - 7pm, which vary by location.
